Every flu program involves a nurse and a physician. (Depending on your selection the physician will either be on-site or available during your flu program).

If you are reserving a program for a company with a medical department and your nurse will be giving the immunization, we will need a copy of:

bulletDoctor's license

If you only have a nurse we will provide the physician.
